LCCAP Health Services Office requires any newly enrolled child to have complete physical and immunization record BEFORE starting. A complete physical includes screening results, immunizations, confirmation that the child is free and clear of communicable disease, any formal diagnosis, and any health condition that a child may have (rescue medication).
